Out of Adjustment: causing difficulty to operate switch – switch lever stops unexpectedly or springs.

Not Maintained: the switch is not lubricated and is difficult to operate.

Obstruction: the switch points or throw rod hangs or catches on a plate, bolt or other piece of equipment.

Foreign Objects: a foreign object is between the switch points or around/under the throw rod and the switch cannot be operated (thrown) in a normal manner.
